Replaced use of custom 'baseUrl' helper with 'url'
...
Also changed up how base URL setting was being done
by manipulating incoming request URLs instead of
altering then on generation.

Potential breaking config change: `GRAVATAR=false` replaced by `AVATAR_URL=false`
